It is necessary to fill out the written application form for reporting the loss of specimen seal wherein the serial number and quantity of the shares shall be specified. Then, the applicant shall submit the application form, together with the applicant's ID card and a photocopy thereof, new specimen seal and stock certificates (for shares bought in from the exchange market or for which the ownership transfer is registered), to the Company personally. (If the applicant who is a natural person files the application via another person or mail, he/she shall also submit the certificate of specimen seal issued by the household administration office or the original of his/her ID card. If the juristic shareholder shall attach the notice of application). Except the new specimen which shall be effective on the same day of declaration, the others shall become effective as of the following day.

(1)Where the application is filed via mail, the domestic shareholders shall attach the originals of their ID cards, while the foreign shareholders shall attach their ID certified documents issued by the overseas agencies of this country.

(2)Where the shareholder appoints another person to file the application, the appointee shall be the national of this country and submit the documents referred to in the preceding paragraph (1) together with his/her ID card and the proxy.

Then, the application form, new specimen seal and stock certificates (bought from the securities market or for which the ownership transfer is registered) shall be submitted to the Company to complete the procedure for change of specimen seal.
