The shareholder's communication address is based on the particulars specified in the shareholder's certificate of specimen seal. Nevertheless, for the shareholders who proceed with registration of ownership transfer via central depository account, their address shall be based on that informed by the central depository company since they have not yet filled out the certificate of specimen seal. Where the address disclose by the shareholder does not match the one informed by the central depository company, the most update address registered by the shareholders at this Company shall apply. The shareholder referred to in the preceding paragraph shall insert the communication address or domicile into the application form and then send the form back to this Company.

It is intended to prevent others to forge the photocopy of the shareholder's ID card and specimen seal by means of the mailed notice, and from claiming the stock on the ground of the forged certificate of specimen seal. Therefore, in order to protect the shareholders from suffering loss in interest and right, the ID No. indicated in the notice should be incomplete.

Nevertheless, if the shares include central depository shares, the information provided by this Company will not necessarily be correct, since only when the ownership transfer is suspended, such as shareholders' (extraordinary) regular meeting, ex divide and ex warrant, the central depository company will transfer the information to this Company. Therefore, if the securities supplier has traded in this period, the information will be updated accordingly. The number of shares shall be subject to the balance of the central depository shares registered by the shareholder at the securities supplier's place.

You have to check with the Company whether the check is cashed or not. If not and the due date has not yet expired, this Company will provide the check number and amount to enable the shareholder to report the loss at the paying bank. If the due date of the check has expired, the shareholder shall apply for re-issuance of the check with this Company pursuant to the relevant requirements.
